Intake

To begin supervised visitation services with DC Visitation, all clients must complete an intake form. This form helps us gather important details about your case, visitation needs, and any specific court requirements so we can provide the safest and most appropriate services for your family.

• Contact information for both parents or parties involved
• Details about the parties involved in the visits
• Court order information
• Scheduling preferences and availability
• Safety concerns, special instructions, or visitation restrictions
• Emergency contact information
• Consent to policies and service terms

• Remain in compliance with court or agency expectations
• Understand your family’s needs and any unique circumstances
• Can assign a supervisor who is the best fit for your case
• Protect the emotional and physical well-being of all participants

1.   We  will review your form within 24 to 48 hours.
2. A staff member will contact you to schedule your intake meeting (via phone, Zoom, or in-person).
3. After the intake is complete and the fee is paid, visits can be scheduled.

There is a one-time $45 intake fee due before first visit is scheduled.
